School Based Health Care
A joint project between Manchester Memorial Hospital, COLLY, and Leslie County Elementary Schools to bring more consistent health care to school children
Services
Each school clinics provides daily sick care and follow-up care to children as well as well-child screenings and sports physicals. To receive these services, parents must complete and return permission paperwork at the beginning of each school year. Children cannot be seen for any reason if this paperwork is not completed. This paperwork can be picked up at each school's clinic.
Cost
Visits with the LPN/RN:
- No cost
- Will not be billed to insurance
Visits with the FNP/PA:
- May cost
- May be billed to insurance
- May receive a bill for a copay based on your insurance policy
- Will cost
- Will be billed to insurance
- May receive a bill for a copay based on your insurance policy
- $15 at time of physical
Families with no insurance may be responsible for the cost of their child's visit, but a sliding fee scale is offered. See clinic staff for details.
